    Everyone enjoy their summer break!

    Remember to be active, healthy, and fit!  Keeping safety first of course!

     

     

     

    The first day of school is Tuesday, September 6 for students.

    Physicals are required for new entrants to the district, students in Pre-K, Kindergarten, grades 1, 3, 5, 7, 9 and 11. Please schedule your child's physical as soon as possible, since the cut off date is 12 months prior to the beginning of the upcoming school year. Please have the form completed by the physician and signed by a parent/guardian. Bring, send or fax the form to my office by the first day of school.

     

    Proof of a dental examination is requested for new entrants to the district, students entering Pre-K, Kindergarten, and grades 1, 3, 5, 7, 9 & 11. The certificate below needs to be handed in once the examination is completed and has been signed by both the dentist and the parent/guardian.

     

     

     

     

    Be sure that your student is updated with the most recent immunization requirements for school re-entrance. Their are NEW requirements that will start September 1, 2022. 

     

    ANY STUDENT REQUIRING MEDICATION AT SCHOOL.......A doctor's order with instructions for use and parental signature is required for ALL medicine given during school hours.  This includes over the counter and prescription medications.  This form is located on the  School health office web page under Medication Authorization.

    An adult MUST deliver medication(s) to the Health Office in its original container.  Students are not allowed to carry any medication to school.
