Medical Matters 

MEDICAL INFORMATION:

•          Physicals are required by New York State for new entrants, students in Pre-K, Kindergarten, Grades 1, 3, 5, 7, 9 and 11. Please schedule your child physicals as soon as possible. The cut off date is 12 months prior to the beginning of the upcoming school year, see form below. When completed please bring, mail, or fax the form to the health office.  The fax number is 757-9955.

•           Proof of a dental exam is requested for new entrants; Pre- K, Kindergarten, 1, 3, 5, 7, 9, and 11.  The exam must be completed no more than one year prior to the current school year, see form below.  A certificate needs to be handed in once the examination is completed. 

•          During the school year, a health screening will be done by the nurse. This includes vision, hearing and scoliosis. Each grade has a different requirement from New York State.  Parents will be notified regarding any significant concerns by letter in the mail.

•       The 2022-2023 school year, Elba Central school will be required to report to the NYS Department of Education your child's BMI (Body Mass Index). However, if you do not want your child's BMI reported, please provide in writing to the Elba Central School health office your request.
